Please read these Terms of Service (“Terms”) carefully before using the mobile wound care services provided by La Casa Wound Specialist (“La Casa,” “we,” “us,” or “our”). By accessing or using our services, you agree to be bound by these Terms. If you do not agree with any part of these Terms, you may not use our services.
1. Services Provided
La Casa Wound Specialist provides in-home and facility-based mobile wound care services. Our services include, but are not limited to, wound assessment, personalized treatment plans, application of advanced wound care dressings, debridement, infection management, compression therapy, offloading techniques, and patient and caregiver education aimed at optimizing skin wound healing and wound recovery. We are not an emergency service. For medical emergencies, please dial 911 or visit the nearest emergency room.
2. Eligibility for Services
Our services are available to individuals within our service area (Southern California, primarily Los Angeles and Orange Counties) who have a medical need for wound care and have a valid referral or physician’s order when required. By agreeing to these Terms, you confirm that you are at least 18 years of age or are the legal guardian of the patient receiving services.
3. Patient Responsibilities
As a patient or caregiver, you agree to:
- Provide accurate and complete medical history and information relevant to your wound care.
- Follow the treatment plan provided by La Casa clinicians.
- Notify La Casa promptly of any changes in your condition, medications, or any issues with your wound or treatment.
- Ensure a safe and accessible environment for our clinicians to provide services in your home or facility.
- Cooperate with La Casa staff during visits.
- Be available for scheduled appointments or provide timely notice for cancellations or rescheduling.
- Understand your financial responsibilities related to the services provided.
4. Appointments and Cancellations
We understand that schedules can change. If you need to cancel or reschedule an appointment, please notify us at least 24 hours in advance by calling 310-810-6554. Failure to provide timely notice may result in a cancellation fee. Our team will make reasonable efforts to accommodate your scheduling needs.
5. Payment and Billing
You are responsible for the timely payment of all services rendered by La Casa Wound Specialist. We will verify your insurance coverage and inform you of any estimated co-payments, deductibles, or non-covered services prior to treatment. While we strive to provide accurate estimates, the final cost may vary based on the actual services provided and your insurance plan. 6. Privacy and Confidentiality
Your privacy is paramount to us. We handle all Protected Health Information (PHI) in strict accordance with the Health Insurance Portability and Accountability Act (HIPAA) and other applicable federal and state privacy laws. Our Privacy Policy, available on our website or upon request, details how we collect, use, and disclose your information. By agreeing to these Terms, you acknowledge that you have reviewed and understand our Privacy Policy.
7. Limitation of Liability
La Casa Wound Specialist provides professional medical services. While we strive for optimal patient outcomes, no medical treatment can guarantee specific results. La Casa, its clinicians, employees, or affiliates shall not be liable for any direct, indirect, incidental, consequential, special, or exemplary damages arising from your use of our services, except in cases of gross negligence or willful misconduct.
8. Governing Law
These Terms of Service shall be governed by and construed in accordance with the laws of the State of California, without regard to its conflict of law principles. Any legal action or proceeding arising under these Terms will be brought exclusively in the federal or state courts located in Los Angeles County, California.
9. Changes to Terms
We reserve the right to modify these Terms at any time. Any changes will be effective immediately upon posting the revised Terms on our website or providing notice to you. Your continued use of our services after any such changes constitutes your acceptance of the new Terms.
